By completing this registration form, I authorize Transfiguration staff to contact appropriate medical personnel in the event of a medical emergency concerning my child(ren).
I give Transfiguration staff permission to give my child(ren) over-the-counter (OTC) medication.
To promote and chronicle activities, the Archdiocese of Atlanta and/or Transfiguration may record students (interviews, images, and/or video) for internal and external use. Content may be reprinted in
The Georgia Bulletin or other media for public dissemination, including but not limited to film; video; television; radio; newspapers; websites and online platforms; and social media networks, including but not limited to Facebook, Twitter, and Instagram.
I give permission for my child(ren) to be recorded. I release and relieve the Archdiocese of Atlanta and Transfiguration from any responsibility or liability for any claims arising from the publication or reproduction of any photographs or interviews in any news or other media. I waive any and all right to inspect or approve the finished images, video, or printed matter that may be used in conjunction with any image or video, or to approve the eventual use for which it may be applied. I understand that photographs, videos, and /or interviews are being done with the knowledge and approval of the parish.
The Archdiocese of Atlanta requires all students (kindergarten through grade 12) to participate in safe environment training. Children are taught that God is a loving, listening, caring presence in their daily lives and that they should feel secure and safe in sharing information with a trusted adult about problems or people that worry them. Sessions are age-appropriate and parents are welcome to attend. Dates / times will be published in the parish bulletin. For more information, visit the Faith Formation page of our website.

Transfiguration follows the Archdiocese of Atlanta's Social Media Policy and Guidelines for the Use of Social Networking Sites with Minors for contacting youth (grades 6-12 only) via social media, text, phone, and/or email.
I give permission for my grade 6-12 child(ren) to be contacted. I understand I can request the same communications provided to my child(ren) and that it does not have to be via the same technology (for example, if child receives a reminder via Twitter, parent can receive it by email).
The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is extremely contagious and is believed to spread mainly from person to person through respiratory droplets when an infected individual coughs, sneezes or speaks. As a result, government agencies at all levels and federal, state and local health agencies recommend social distancing and have placed limits on the congregation of groups of individuals.
Transfiguration has put preventative measures in place to reduce the spread of COVID-19; however, the parish cannot guarantee that you or your child(ren) will not become infected with COVID-19. Further, attending activities on the campus of Transfiguration could increase your risk and that of your children for contracting COVID-19.
While Transfiguration will make all reasonable efforts to lower the risk of COVID-19 exposure and spread at the parish, the parish is unable to provide any guarantee that students or their families will not be exposed to or infected by COVID-19.
By enrolling your child(ren) in and attending parish faith formation, you acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that you, your child(ren) and or other family members may be exposed to or infected by COVID-19. It is expected that students and other family members will follow the preventative measures and guidelines implemented by Transfiguration including not coming to the parish premises if demonstrating any signs or symptoms of COVID-19.

When participating in digital and/or virtual learning at Transfiguration, there are basic technology requirements, such as access to digital devices and internet service, that are required for a successful learning experience. Transfiguration will try to support those enrolled who do not have access to these basics by providing printed materials, resources for reduced cost internet, etc. Families receiving this assistance may not have a learning experience equivalent to others enrolled in the same class.

I UNDERSTAND THIS IS A FAMILY PROGRAM AND WE WILL ACCOMPANY MY/OUR CHILD TO EACH SESSION AND COMPLETE THE FAMILY AT HOME ACTIVITIES. It will be beneficial for the entire family to participate together.
Submission of this form does not guarantee placement in class. No child will be denied Faith Formation because of an inability to pay.
Registrations will be processed when tuition is paid in full OR a Payment Plan/Tuition Assistance form is received.
